Five Why Access Options
A "Five Why analysis" may be added to any Corrective Action entry. In the image below:
A.Add or edit a single "Five Why analysis" by opening a CAPA log record and move to Tab 6);
B.Access all "Five Why analysis" by click on the Five Whys? button.
From Tab 6) Five Why (on CAPA form)
In the CAPA form, Tab 6) Five Why image below:
A.Tab 6) contains one or "Five-Why Analysis" for the currently opened CAPA record.
B.A message field to identify which Five Why and counter measures are determined to be the "root cause" statement.
C.The Summary Comments from the Five Why analysis may be applied to the Tab 2) "Cause Statement" of the currently opened CAPA record.
D.Showing the Summary Comments from the Five Why record highlighted in the table [ E ].
E.Five Why records relevant to the opened CAPA record.

A.Select from the General tab; 1) Why?; 2) Why?; 3) Why?; 4) Why?; 5) Why?
B.Which "Why" contributes most to the "Root Cause"?
C.Contains the Summary Comments from Five Why analysis
D.Attachment relevant to the analysis
E.The Why Question and Answer, along with any counter measures or aspects noted.
